The parent or legal guardian of the child they wish to exempt from vaccination needs to contact the Immunization Program at the Arizona Department of Health Services. They may be telephoned at 602-364-3636. Request a religious exemption form be either mailed or faxed to you. Fill out the form and give it to the daycare or school.
