The parent or legal guardian of the child they wish to exempt from vaccination needs to fill out the affidavit on the back of the blue card that you wish to exempt your child from immunization. This is done when you enroll your child in school!
Daycare is a little different. If your child is in a private daycare than the immunization issue is between the parent or legal guardian and the daycare. If your child is in a public affiliated daycare than the philosophical exemption applies to daycares the same as it does to public schools.
