The parent or legal guardian of the child they wish to exempt from vaccination needs to contact the State of Deleware Division of Public Health, Immunization Branch at 1-800-282-8672 and request a religious exemption form be mailed to you. They will fax or mail you the form. Once you have the form, simply fill it out and give it to the daycare or school upon registration.
