The parent or legal guardian of the child they wish to exempt from vaccination needs to fill out the Certificate of Exemption From School/Daycare Immunization Requirements form (Click on the link below for the form) and have it notarized. The form must then be mailed to the New Mexico Immunization Program (preferably by certified mail). Their address is:

They will review and sign the form, and mail it back to you. When you receive the form back from the Immunization Program, give it to the daycare or school upon registration. This process needs to be done every school year in order for your child to be exempt from immunizations.
