The parent or legal guardian of the child they wish to exempt from vaccination needs to obtain a moral (philosophical) exemption or religious exemption form from the school or daycare upon registration. Just sign on the form whether you want religious or personal exemption. The forms can also be downloaded for free at the Vermont Department of Health, Immunization Program at: http://healthvermont.gov/hc/imm/schoolentry.aspx and click on School Immunization Exemption Form. They can also be telephone at 802-863-7241.
