The parent or legal guardian of the child they wish to exempt from vaccination needs to obtain a philosophical or religious exemption form from the school or daycare upon registration. Just sign on the form whether you are requesting a religious or personal exemption. In the event the school or daycare does not have the form, you can download the form by clicking on the Wisconsin Philosophical/Religious Vaccine Exemption Form link found below.
